Risk Management for Work at Height

Graham Marshall - Wednesday, May 25, 2011
As an Australian Safety Practitioner, it is simply astonishing that OSHA Regulations in the USA allow for "free-climbing" in ways shown in this video!

Below is a reference from  Section 1910.269(g)(2)(v) of OSHA regulations which states:

"Fall protection equipment is not required to be used by a qualified employee climbing or changing location on poles, towers, or similar structures, unless conditions, such as, but not limited to, ice, high winds, the design of the structure (for example, no provision for holding on with hands), or the presence of contaminants on the structure, could cause the employee to lose his or her grip or footing".

The worker is more than 550 meters above ground surface on an antennae with no fall restraint.  Astounding!

You'll be scared!

Buncefield Terminal Explosion

Graham Marshall - Thursday, March 17, 2011

In the early hours of Sunday 11th December 2005, a number of explosions occurred at Buncefield Oil Storage Depot, Hemel Hempstead, Hertfordshire, UK.

At least one of the initial explosions was of massive proportions and there was a large fire, which engulfed a high proportion of the site.

Over 40 people were injured but fortunately there were no fatalities.

Significant damage occurred to both commercial and residential properties in the vicinity and a large area around the site was evacuated on emergency service advice.

The fire burned for several days, destroying most of the site and emitting large clouds of black smoke into the atmosphere.

Enclosed here is an interesting video from Gexcon that illustrates the role played by neighboring trees in escalating the size and scale of the explosion.  It should be seen by anyone with an interest in process hazard management at fuel storage terminals or refineries.

Further information about the Bunsfield Oil Storage Depot can be found by clicking here.

Graham Marshall - Wednesday, January 12, 2011

Today's simple safety "one per-center" action is to inspect and verify the fitness of ladders being used in your workplace.

Enclosed here is a short video to help get you thinking about what to look for with ladders or to use in a tool-box talk.
